Texas law requires that your veterinarian approve your horse to receive chiropractic care. Have your vet complete and sign the areas of your form highlighted in YELLOW.

E-mail the completed CONSENT FORM, signed by your vet, by using the E-MAIL CONSENT button. Send a scanned version of the form.

Once you have submitted your CONSENT FORM, signed by your vet, select your appoint type below to begin the scheduling process.
Please do not schedule your appointment before we receive your CONSENT FORM. Appointments scheduled without CONSENT FORMS will be cancelled. We apologize for that formality but Texas requires us to have your vet's approval and their supervision in order to help your horse.
